Format in Word under test result

Hello, I am looking for a person to help me create a professional looking Word document template format. In it will be placed sample patient results, currently I have such a file exported from PDF to Word. Unfortunately, as a result of the conversion, the formatting of the document, including tables, has become scattered. I need , the formatting in Word to be functional and aesthetically pleasing, including correct formatting of tables and proper layout of text. So that after entering data , the tables do not crash.

To summarize: what I care about is to create a formatter in word, to preserve the existing layout of the document, so that the tables do not crash when the data is completed, to ensure that the tables are properly scaled and centered, the document should be a bench to edit.
