Production of an active catalog with an order form in PDF format

Good day,

We would like to commission an active catalog with an integrated order form as a pdf file of approx . 6-8 pages. The file should have the ability to select options both from pick lists and manual entry. A desirable option would be automatic switching from page to page after the entire page has been completed.

The directory should have information such as:

1. company logo

2. product outline

3. sections such as:

- product type

- product name

- product model

- product code

- height of the product

- width of the product

- length of the product

- equipment and quantity

- color selection

- additional equipment and quantity

- box for entering additional information

- terms and conditions

- RODO

- download button

- other fields (to be specified)

PDF file must be fillable on a computer.
