I need to change forms on the website. Additional fields on existing forms (2) additional forms (2), changes in the database for data registration, in total, about 40 fields. Report comparing various data from forms - 1 new 2 for change. Test support and troubleshooting, minor corrections after testing - up to 10 changes.

Implementation of changes to production, post-implementation telephone support within 2 weeks.

In the future possible additional expansion and care of the project.
