Simple Online Service Request Form

Good day,

I need to make a simple Online service request form for our service customers. Below I describe the concept to give shape to the project:

The company is engaged in the installation and service of mowing robots, which generates the need to perform systematic service activities for these products.

I want to be able to send the customer a link to the service request, under which he will find a form to fill out.

After filling out the form, the request should be sent to the appropriate e-mail box, which is handled by service technicians so that they can contact the customer directly and arrange an arrival date.

The form does not need to have a database to store and segregate requests. It is enough for it to send full information just to our e-mail box.

Data to be filled in by the customer in the form:

Name

Email

Contact phone

Installation address of the robot

Manufacturer

Model

Serial No.

For the above three points, a photo of the robot nameplate can be sent as an alternative.
