Registration form on the website

We are looking for a contractor for a simple registration form on the website.

The form is to collect (and store in the database) personal data:

1. Name

2. Surname

3. email address

4. mobile phone number.

What is important for me, after entering all the data (all 4 fields are required) and pressing the "Registration" button, a PIN code (4 digits) should be generated from the database and sent via our own SMS gateway to the provided mobile phone number.

The user should enter it in the next field and if the PIN code is correct, he/she may complete the registration process by checking the checkboxes with consent to the processing of personal data and accepting legal provisions on how his/her data will be used.

For those interested, I can send the message format that should be generated for sending to the url of the SMS gateway.
