I will order the creation of a portal for ordering medical consultations.

Requirements:

- Next.js

- Informational home page with sections: Main information what we do, tiles with redirection to specific forms, our 3 USPs, how it works, FAQ.

- Form for ordering consultations with e-prescription

- Form for ordering a consultation with an after pill

- Form to order consultation with e-prescription for contraception

- Form to order consultation with e-prescription

- Form to order consultation with referral

- Option to select express option for a surcharge

- Integration with PayU payment system

The portal should have a UX exactly like, for example, prescription express dot pl.

Integration with the office system will be on our side.
