Forms - Service request

Good day,

I would like to commission the creation of a contact form on the website (already existing), I invite you to make an offer after a complete review of the order (including the attachment).

After accessing the site:

The customer will see three buttons:

1. warranty notification

2. post-warranty notification

3. periodic inspection offer

Assumptions:

A. If the customer sends the form, we receive all the data contained in the form by e-mail to [email protected], the question is what about the attachments, will they also be in the form of an attachment? This would be best for us while I see a problem about which below in my question.

B. Each form has a button, "Back" which redirects back to the page with three buttons.

C. Fields with a red asterisk indicate that they are not mandatory.

D. We will do the styles at the end once the form is functional

Issue to be resolved:

I would like to make the service request process for customers as simple as possible, and I assume that attachments may be an issue here. The customer may want to record a video or take a picture while filling out the form and this is the possibility I would like to give him, and as we know currently phones take photos and videos of great weight. Alternatively, we can use, for example, a Google Drive type space where attachments from submissions will be stored for 30 days then deleted automatically, this will allow us to accept larger attachments.
