You need a form on fentiks.co.uk, here an example from another of our sites https://grupasilesiaszkolenia.pl/wp-content/uploads/2020/01/Wnioski.pdf ,in which the customer can enter their data and download them in pdf, and with us this data will be in excel. Data to be entered only on the first page, the second page is to print out complete. Below the current applications that have more points.
