A website with a form and two databases.

We are looking for a company or freelancer who will make for us a website design that will be 99% mobile playable.

The homepage will depict the whole project and refer to the form which will be the main value of the project.

The form will be created with 8 - 10 stages which will have to be filled out with: yes or no answers, multiple choice selections, custom entries and the like.

There will also be more advanced functions:

The first function that the form will have is to generate a link and send it via SMS gate ( explanation: the form will begin to fill in one person who will create the original number of the "case" then the link will be generated to other 2, 3 or 4 people from which each of these people will have to confirm the entered some data by the person sending or change the data and send for confirmation to others. If this is not understood I will be happy to explain in a private message with an example )
