Creation of a patent law application form

I need to make an application form for patent law.

The form should consist of two parts.

The first part - the client gives data (Contact details of the applicant and the contractor), place of protection (Poland, Eu, other countries), division of the case (Invention, trademark, utility model, industrial design).

When the trademark - depending on the categories and their number should show the amount of valuation (According to the conversion factors that I will provide. These data are on the website of the Patent Office, it is good if the form can be connected to them).

When a utility model or invention is selected - space is needed for a description of the thing with a possible attachment (Preferably any file, primarily JPG PDF text)

When an industrial design is selected - how many variations of the mark, also the possibility of attaching a file.

Part Two - Space for attachments. All data of the object, other data (company, tax ID, address). Data of the creator and applicant separately (For invention, utility model and industrial design).
